Welcome to the Health Center at Fox Run. Our 54- acre campus has everything you need from relaxation to rehabilitation. You will see friendly faces at every turn, as our compassionate staff personally cares for each Community Member’s well-being and quality of life. At Fox Run at Orchard Park, we help you plan for your next steps – wherever your journey may lead. The multiplex Health Center at Fox Run has assisted living, memory care, and skilled nursing available to you, should your needs change. Our Health Center has all your medical appointments in mind with our fully equipped, state-of-the-art physical and occupational therapy center, dental office, and on-staff medical doctor. A podiatrist, psychologist, psychiatrist, and massage therapist are available to meet one-on-one with you as well.

Our Offerings

Assisted Living

Assisted Living at Fox Run has comprehensive services with a personal touch. Our compassionate, dedicated staff are committed to providing a safe, caring environment that ensures peace of mind for each and every Community Member. With emphasis on personal space and comfort, our 34 Assisted Living accommodations include a private bath with shower, a kitchenette, individually controlled heating and air conditioning, and a flat screen television.

Memory Care

With 18 specially designed Memory Care suites in the LPCiminelli Memory Care Neighborhood, the New York State Certified Memory Care Program for Alzheimer’s Disease and dementia at Fox Run is designed to accommodate people living with cognitive impairments. The components of these programs serve to enhance the self-esteem of Community Members and minimize the isolation that accompanies memory loss. Our specially trained staff develop unique care plans for each Community Member which are implemented in a professional, thoughtful, and caring manner.

Skilled Nursing Care (24-Hour Care)

Fox Run offers a 60-bed Skilled Nursing center with exceptional services held to the highest level of quality care standards. Our four distinct neighborhoods allow for a unique opportunity for personalized attention and individualized assistance, giving each Community Member an active voice in how they spend their time. The Clemet & Margot Ip Rehabilitation Center, newly renovated and state-of-the-art, is also available for those Community members who require therapy on a temporary basis.

The Fox Run at Orchard Park Physical Therapy program works with one goal in mind — to bring health and happiness back to Community Member’s everyday lives. Through engaging exercises and techniques, our highly-skilled team of Physical Therapists works with individuals to create personalized programs tailored to individual care needs. By working with a Physical Therapist, Community Members can expect to strengthen their mobility, ease chronic pain, and much more — all with convenient scheduling options.

Meet the Team

Tracy Swyers, PT, MSPT, GCS

Board-Certified Clinical Specialist in Geriatric Physical Therapy

Physical Therapy Supervisor

Certified by the American Physical Therapy Association, Tracy A. Swyers is part of an elite group of professionals specifically certified in geriatrics. As the Physical Therapy Supervisor at Fox Run, Tracy is designated as a Board-certified Clinical Specialist in Geriatric Physical Therapy, as well as a Diplomate of the American Board of Physical Therapy Specialists.

Tracy Swyers, joined the Fox Run at Orchard Park team in 2014, bringing over 26 years of clinical experience, including 21 years as a Rehabilitation Program Director. Tracy manages a team of 10 therapists at Fox Run, in addition to educating Community Members and staff on how to best utilize PT services through educational programs related to physical wellness, balance, mobility, safety awareness, falls, and daily function. Tracy’s additional experience in home health care, chronic pain management, outpatient orthopedic rehabilitation, and instruction at the collegiate level for the physical therapy assistant program, makes her the ideal care professional to lead the PT team at Fox Run. 

Tracy attended Ithaca College and earned both a Bachelor of Clinical Science Degree and a Master’s Degree (summa cum laude) in Physical Therapy. She is a member of the National Phi Kappa Phi Honor Society and was an NCAA All American athlete in volleyball. Recognized for her volleyball career, Tracy was inducted into the Ithaca College Athletic Hall of Fame in 2012.

In 2011, Tracy became a “40 Under 40” recipient in Buffalo Business First.

Culture of Wellness

The dynamic Creative Arts Staff is committed to providing engaging entertainment, stimulating art projects, and fitness activities on a regular basis. The Health Center at Fox Run also boasts a home-like and familiar dining setting for all three meals per day. Our case manager is involved with each Community Member and their family to help adjust to changes and monitor progress along the way.

LeadingAge New York

Fox Run at Orchard Park is also an active member of LeadingAge New York. This organization was founded in 1961 and was formerly known as the New York Association of Homes & Services for the Aging (NYAHSA). It represents not-for-profit, mission-driven, and public continuing care providers, including nursing homes, senior housing, adult care facilities, continuing care retirement communities, assisted living, and community service providers. LeadingAge New York provides a vehicle for sharing information and research as well as providing advocacy and regulatory support that benefits its 600 members.
